Description

For Patients

Patients must have valid identification (picture ID preferred) , written orders, or a completed requisition form containing the following information, before coming to the Laboratory:

1. Name and Unit Number
2. Type of Specimen
3. Tests requested
4. Provider’s full name, telephone number, and location
5. Diagnosis Code(s)

For Providers

If a provider wishes to order a test not performed by the MG West Lab, please check off the test under the appropriate Laboratory section on the Laboratory requisition. All Blood Transfusion Service (BTS) tests must be ordered on the appropriate BTS requisition. The Laboratory will collect and forward specimens to the appropriate laboratory for testing. Providers who collect specimens in their offices or on home visits, must label the specimen with the patient’s name and unit number, place it in a plastic bag, and secure a completed requisition around the specimen with an elastic.

Criteria For Specimen Refusal

• Specimen not labeled with patient’s name and unit number.
• Name on specimen and requisition do not agree.
• Specimen is more than 24 hours old.
• Quantity Not Sufficient (QNS) for testing.
• Specimen unsuitable for testing.
• Date of Specimen unknown.
• Specimen arrives without a requisition.
• Improperly sealed or leaking container.

Results are accessible by Epic and are available by calling 617-724-LABS (5227). Tests performed at the Mass General Waltham Laboratory are offered on a routine and STAT basis. The use of STAT is reserved for tests that are for urgent care.
